Youth Summer Camp

About This Event

I, the undersigned parent or legal guardian, hereby give permission for my child to participate in the Islamic Society of Greater Charlotte (ISGC) Youth Summer Camp and all related activities, including educational programs, sports, recreational activities, and field trips. I understand that participation in camp activities may involve certain risks, including but not limited to minor injuries, accidents, illness, transportation-related incidents, and other unforeseen circumstances. I voluntarily assume all risks associated with my child's participation. I release, waive, and discharge the Islamic Society of Greater Charlotte (ISGC), its board members, employees, volunteers, instructors, organizers, agents, and representatives from any and all claims, liabilities, damages, losses, or expenses arising from my child's participation in the camp, except in cases of gross negligence or willful misconduct. I certify that my child is physically able to participate in camp activities and that I have disclosed any medical conditions, allergies, medications, or special needs that may affect their participation. In the event of a medical emergency, I authorize ISGC staff and volunteers to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred. I grant permission for photographs and videos of my child taken during camp activities to be used by ISGC for promotional, educational, and community purposes, including social media, websites, newsletters, and printed materials.
